  Community Care Assessment and Care Planning Training Program

NARI has offered a successful training program for community care assessment staff since September 2001. The program is designed to meet the professional development needs of community based aged and disability assessment staff and case managers working both within the HACC program and other programs catering for the needs of older people or people with disabilities. The program incorporates the principles of person-centred care which supports independence, as reflected in the HACC Active Service Model. Key areas include assessment of decision-making capacity; goal-setting, development and implementation of care plans; and communication skills for understanding individual needs and building effective care partnerships. The two-day program is conducted in the metropolitan area approximately twice yearly, and is run “off-site” as requested in regional areas. NARI can also tailor the program for individual organisations to address specific training issues for their staff. The next metropolitan program is likely to take place in August – September 2008.

  Falls prevention training programs

  SPLATT Attack: Falls Prevention Expo

The SPLATT attack training session was initially developed for staff in the sub-acute setting. The program has since been modified and applied to the acute, residential and community setting. This multidisciplinary, interactive and experiential training program is an informative and fun method of learning. A multidisciplinary team of clinicians, with extensive falls prevention knowledge, run a series of workstations that staff work through in small groups. Each station takes about 10 minutes to complete and the total training takes about an hour.

  Priority of Access (POA) tool training

In 2002-03 NARI developed a training program for Western Metropolitan Region Local Government assessment staff. Feedback from training lead to development of a revised POA tool (POA2). The training report and modification to the tool is available here ....

In 2003-04 the Northern Metropolitan Region Local Government HACC agencies funded NARI to undertake training and further trialing of the tool. Minor modifications were made to the tool leading to POA3. The training report and development of POA3 is available here ....

Further training in use of POA3 has been conducted in 2004 with the City of Monash and the Loddon Mallee Region.
