Person-centred health care is the "treatment and care provided by health services [that] places the person at the centre of their own care and considers the needs of the older person's carers" (Victorian Government Department of Human Services, Improving Care for Older People Policy, 2003).
This website aims to:
- provide easy access to examples of resources and initiatives that may be used to improve person-centred health care
- reduce the likelihood of health professionals expending time and resources “re-inventing the wheel”, and
- provide an ongoing forum for people to share ideas/resources.

This website is part of a project undertaken by the National Ageing Research Institute (NARI) which aims to:
- assess current practice in Victorian health services in relation to the involvement of older people and their carers in their health care
- identify current good practice in person-centred health care, both nationally and internationally
- develop and trial resources to support the implementation of person-centred practice in Victorian health services, and
- evaluate the impact of these resources on practice with clients and carers, direct service providers and service managers.
